The Indiana Occupational Safety and health Administration has fined the Winnebago plant in Middlebury $10,000 for workplace violations after an employee was run over and killed by a forklift in April.

The coroner said 54-year-old Rick Schlabach died of blunt force injuries in the accident at Winnebago Industries on 14th Street.

Two serious violations were noted: A windshield was installed on a forklift, which affected it's operation, and the forklift wasn't operated at a safe speed.

Winnebago was given until August 22 to fix those issues.
